My Caregivers Assessed Fall Risks At Home Before I Arrived
I came home to a healthy respect for the Fall Risks my Caregivers had identified and took care to remove after their collaboration with my Acute Rehab team. In this Quick Guide we have detailed the many potential Fall Hazard associated with the movements of a recovering Stroke Survivor and their Activities Of Daily Living (ADLs).

My Need For Mobility Began Immediately On Returning Home
On my arrival home from the hospital, my Caregivers had purchased and had ready my basic "grannie" walker. Just in case, they also had a transport chair on hand with the likelihood that the long trip home from Vallejo might have reduced my energy.
We knew toileting would be an immediate need on my returning home.
Coming Home From Acute Rehab Meant Being Ready
We were preparing for my homecoming while still in Acute Rehab. There were many areas of our home, and the potential for transporting me to medical and rehab appointments we had to address. After safety, our attentions were on being able to support my Activities For Daily Living (ADL). Toileting was at the top of this list as those functions could not be put on hold. Keeping me active and engaged in my recovery was a goal I monitored myself, but this required the support of my Caregivers from the very beginning of my at-home recovery.

Maintaining My Good Health
My return home meant my Caregivers had to replicate the routines and watchfulness delivered by my hospital teams at Kaiser. To keep me in good health our at home efforts were monitored by my Principle Physician and my assigned Rehabilitation Physician. Maintaining my good health was for us a combination of monitoring and controlling my blood pressure, watch my diet, and institute some form of daily exercise suitable for someone incapacitated on one side and residing in bed or my favorite recliner.
